KorvaLabs, Inc. will bill the insurance provider 100% of the charges. If the health plan instructs us to bill the patient for co-payments, co-insurance, and deductibles then we are required by law to bill accordingly. These amounts are determined by your insurer and not by KorvaLabs, Inc.
Your insurance company will be billed for the services provided. Upon processing of the insurance claim, you will receive an Explanation of Benefits (EOB) from your insurance company. THIS IS NOT A BILL. The EOB simply explains what services were billed to your insurance company and summarizes the payment status of the claim. If you have any questions regarding this EOB, or any correspondence from your insurer, please contact us directly at (818) 401-0080 so we can help you. DO NOT CONTACT YOUR DOCTOR’S OFFICE regarding your drug screen EOB as they will not have the details regarding your claim.
KorvaLabs is committed to completing up to three levels of appeals where allowable by your insurance company and individual plan. We may ask you and your physician to assist in the process as needed. Depending on the insurance company, this can be a very lengthy process. We will keep you informed, as needed, during this process.
In some instances, your insurer will send payment directly to you, instead of us. If you receive a payment from your insurance company, it is your responsibility to pay within 10 days of receipt of payment. Please endorse the back of the check and write “Payable to KorvaLabs, For Deposit Only” and mail to the following address:
Attn: Billing Department
430 S. Cataract Ave.
San Dimas, CA 91773
Failure to send checks to KorvaLabs may result in you being sent to an outside collection agency. To prevent future payments directly to you, please contact your insurance carrier and choose to assign benefits to the rendering provider.

Why am I getting tested?
State and Federal guidelines strongly recommended routine testing for any patient being treated with opioid medication for more than 90 days. Testing for potential abuse or misuse protects both you and the doctor, who’s license could potentially lose his/her license to practice medicine. The physician and or the office does not discriminate based on gender, age, or race so the policy is to test everyone.
What about confidentiality?
All results are only provided to the rendering physician via a secure mean, in accordance with HIPAA guidelines.
What should I expect when I give a sample?
On the say of testing, you will be asked to provide a urine or saliva sample. Be sure to check with the office staff before using the restroom as failure to provide a sample might delay your prescription. The point-of-care cup will detect specific drug classes that can be used by the physician to make on the spot prescribing decisions. The cup is sealed for security and shipped to the lab for a more complex testing and confirmation. Your physician will discuss any unexpected results during your next visit.
How often will I be tested?
Some states have mandated the testing frequency, but your physician will determine the appropriate amount of testing to support your treatment plan.
How much does this all cost?
KorvaLabs will bill your insurance provider directly. Coverage is specific to your healthcare policy and will be explained on your Explanation of Benefits (EOB). The EOB is not a bill. If you receive a bill, DO NOT CALL YOUR DOCTORS OFFICE. Please call and speak with our Billing Department at (818) 401-0088 before making a payment or calling a physician. Our specialists will determine if our Financial or Medial Hardship program can be applied to reduce the outstanding balance.
What is a deductible?
This is the amount that is required by your insurance carrier to be paid by the subscriber before health plan benefits will begin to reimburse for services.
Can I pay cash?
Absolutely, we accept checks, money orders and credit card payments. Send all checks and money orders to: KorvaLabs, Inc. 430 S. Cataract Ave. San Dimas, CA 91773. You can pay by credit card by phone.
What kind of instruments are used to performed at KorvaLabs?
KorvaLabs uses LC/MS/MS technology to identify a wide range of medications at a part per billion levels. This advanced technology eliminates false readings that are observed on point-of-care devices. The sample will also be checked for evidence of tampering or adulteration. Never attempt to alter your sample in any way as it could cause delays in re-filling your medications.
